CNWL NHS Foundation Trust is seeking a LD MH Community Nurse Practitioner to provide specialist nursing support for people with learning disabilities and mental health needs in the Tri-Borough area (Hammersmith & Fulham, Kensington & Chelsea, Westminster).
The role involves independent practice within a multidisciplinary team, travelling across the three boroughs and managing a caseload with annual review of S117 aftercare plans.
