Adult Community Mental Health Nurse — Care, Collaboration & Mentoring

HPFT in Stevenage is seeking a Community Mental Health Nurse to join the Adult Community Mental Health Services Team. You will assess care needs, develop, implement and evaluate programmes of care for a defined caseload of adults with complex mental health and social care needs.

The role includes mentoring student nurses and working to maintain effective liaison with inter-agency partners, families and carers, ensuring high standards of care across in-patient, primary, and community settings.
