Community Mental Health Practitioner – Care Coordinator

Gloucestershire Health and Care NHS Foundation Trust is seeking a Community Mental Health Practitioner (Band 5 or Band 6) to join the Assertive Outreach team in Cheltenham, Tewkesbury and North Cotswolds. The post offers either Band 5 full-time or Band 6 at 0.8 WTE (4 days per week) on a permanent basis.

You will care for adults with severe mental illness, coordinate care, and work within a MDT to promote recovery and social inclusion across community settings.
