ED Social Worker: Discharge & Independence

Cambridgeshire is seeking a Front of House Social Worker to join the Transfer of Care Service in the Emergency Department at Addenbrooke’s Hospital, Cambridge. You will provide social care expertise to promote independence and timely discharges, working with health colleagues and community services to organise personalised, strengths-based support.

You will undertake Care Act and MCA work, assess needs, and develop plans with families and carers to prevent unnecessary admissions and speed up
