Mental Health Discharge Planning Assistant (Hybrid/Home)

Kent County Council is recruiting a Social Work Assistant for the Early Discharge Planning Team. This permanent full‑time role works with adults in Acute Mental Health Hospitals in Kent and some out of area facilities, traveling across a wide geographical area, including from home.

The post requires a Diploma in Health & Social Care (level 2) with progression towards Level 3, and GCSEs in Maths and English.
