Responsibilities
Assist service users with personal care and daily living activities in line with care plans.
Support individuals to engage in social, educational, and recreational activities.
Monitor and report changes in service users’ health, behavior, or needs to the appropriate team members.
Promote independence and encourage positive choices to enhance quality of life.
Maintain accurate and up-to-date records of care provided and observations made.
Work collaboratively with colleagues and external agencies to deliver coordinated care.
Ensure the safety and well-being of service users at all times, adhering to safeguarding policies.
