Autonomous Clinical Practitioner — Hospital at Home Lead

Mid Yorkshire Teaching NHS Trust invites applications for an Advanced Clinical Practitioner to join our Hospital at Home Service. The role delivers patient-centred care at home or usual residence, supports hospital avoidance and timely discharge, and works with a multidisciplinary team.

You will work autonomously with medical, nursing, therapy and AHP colleagues, undertake investigations, assess needs, and lead on clinical decision‑making, education and service development.
