Deliver clinical triage, assessment and treatment for adult inpatients
Provide prescribing, medicines optimisation and escalation support
Contribute to end-of-life care planning and safe handovers
Work across multiple community hospital sites depending on clinical need.
This role is ideal for experienced clinicians registered with the NMC / HCPC / GPhC (or equivalent), with prescribing capability and a passion for delivering high-quality, patient-centred care in a community setting.
You’ll be part of a service focused on providing timely clinical advice, treatment decisions and safe care for patients in rehabilitation, intermediate and palliative care pathways.
