Frailty GP Leader – Home Visits & MDT Care Hub

Dorking Healthcare Ltd. is seeking a Frailty General Practitioner to join the South Tandridge Primary Care Network across Oxted Medical Practice, Pond Tail Surgery and Lingfield Surgery.

The post involves triaging, developing care plans for frail patients, and providing home visits to reduce unplanned admissions. You will lead the frailty hub, work with MDTs, coordinate with Community Matrons, District Nurses, Pharmacists, and other providers, and provide clinical leadership within the PCN.
