Community Nursing Associate — Home Care & Independence

Lancashire and South Cumbria NHS Foundation Trust in the Chorley, South Ribble and Preston area is seeking a Community Registered Nurse Associate to provide in-home and community care as part of our district nursing teams.

The role involves home visits, comprehensive assessments, care planning, medication administration, wound and continence care, and working with GPs and social care to keep patients independent in their own homes.
