Single Point of Contact Referrals Coordinator

Hampshire and Isle of Wight Healthcare NHS Foundation Trust’s Single Point of Contact team in Fort Southwick, Fareham, is the first contact for referrals into community services across Fareham, Gosport and South East Hampshire.

The role involves processing referrals, liaising with GPs and other health professionals, maintaining records, arranging meetings and ensuring timely action plans for patients as they move through community care pathways.
