Community Outreach & Wellbeing Facilitator

Plymouth City Council in Plymouth is seeking a Community Outreach Worker to support the care management process and help adults with a variety of needs promote independence and wellbeing.

You will signpost and facilitate involvement with services and agencies, using research, information and guidance to help people access local community resources.
