Advanced Practice Nurse – Care Home Lead Clinician

The Lytham St Annes Primary Care Network seeks an experienced nurse clinician to operate as an Independent/ autonomous practitioner delivering urgent and proactive care to care-home residents. You will flex between rapid-response assessments, scheduled work and MDT reviews to improve outcomes and reduce hospital admissions.

Essential experience includes post-registration autonomy, care for frail older people with multi-morbidities, and leadership in clinical governance.
