Advanced Clinical Practitioner – Frailty & Community Care

Widnes Highfield Health Ltd is seeking an Advanced Clinical Practitioner to join the Widnes Primary Care Network and deliver an innovative frailty service across GP practices, patients’ homes and care homes. You will perform comprehensive geriatric assessments, coordinate care with a multidisciplinary team and promote anticipatory planning to improve outcomes and reduce avoidable hospital admissions.
