Community GP: Proactive Home-Visit Care

East Shore Partnership is seeking a GMC-registered GP to provide proactive, high-quality care in the community as part of the Integrated Neighbourhood Team. You will manage long-term conditions, dementia/SMI checks, and LD health assessments for vulnerable patients.

The role emphasizes anticipatory care planning, collaboration with nurses, AHPs, social prescribers and secondary care colleagues, and delivering comprehensive documentation within IT systems.
