Hybrid Advanced Practitioner Social Worker – Community Care

Somerset Council is seeking an experienced Advanced Practitioner Social Worker to lead neighbourhood teams across East and West Mendip, Yeovil, South Somerset and beyond. The role involves supervision of social care staff, inter-agency collaboration and driving independence for residents within community settings.

You will work in a hybrid model with at least two days in the office and eligibility for a Skilled Worker visa sponsorship is possible for those already resident in the UK.
