Community Reablement Specialist (Level 2)

Hampshire County Council is seeking a Community Reablement Assistant (Level 2) to support adults over 18 in Hampshire, helping them live independently at home. The role involves initial visits, reablement interventions, goal-setting and progress monitoring within a multidisciplinary team including OT and Sensory Officers.

You will work across the Central Hampshire area (Winchester, etc.), travel as needed, and contribute to rapid discharge from hospital.
