Hospital Discharge Social Worker (Adults) – Hull

Sanctuary Personnel Ltd in Hull is seeking a qualified Social Worker to join the Adults Multi-Disciplinary Team. The role offers 37 hours per week on an ongoing, full-time basis with a strong focus on hospital discharge planning and ensuring safe transitions for adults after hospital care.

You will perform Care Act assessments, collaborate with healthcare professionals, families and agencies, and develop person-centred plans to promote independence and better outcomes for service users.
