Transitional Case Manager – Exeter Community Support

The Canadian Mental Health Association Thames Valley Addiction and Mental Health Services in London, Ontario is seeking a Transitional Case Manager at the Exeter Site to support clients with crisis management, individualized rehabilitation planning and service coordination. You will work with clients to connect to community resources and teach life skills for recovery.

The role requires a health/social service diploma, a valid driver’s license, and a Police Vulnerable Sector Check.
