Outreach Mental Health & Wellbeing Practitioner

Change Mental Health is seeking a Mental Health and Wellbeing Practitioner (Outreach) to join our Central Services team in Scotland. This community-based role supports adults affected by mental illness, enabling them to build confidence, resilience and greater independence.

You’ll work primarily one-to-one with people in their homes and local communities, with opportunities to contribute to Hub-based services when needed.
