Discharge Support & Readmission Prevention Worker

Sheffield Teaching Hospitals NHS Foundation Trust is offering bank shifts with a four‑week release cycle. Pay is £13.45 per hour for 7‑hour shifts, with hours typically between 9 a.m. and 6 p.m.
