Rapid Response Community Nurse – Band 5

East London NHS Foundation Trust is seeking a Band 5 nurse to join the Primary Care at Home team delivering urgent community care in London. You will triage referrals, take histories and perform assessments, and plan care with patients, working with the MDT across unplanned care.

You will have a valid UK driving licence and be required to work a shift pattern of 08:00–20:00 seven days a week.
