Youth Social Care Assistant — Transition & Independence

South Gloucestershire Council is seeking a Social Work Assistant to support disabled children and young people, their families, and networks. You will assess cases, develop care plans, and enable independence using a strength-based model in collaboration with the Pathway to Independence team.

You will travel within the authority and must have access to transport or a valid driving licence; an enhanced DBS check is required for this post.
