Are you the right candidate for this opportunity Make sure to read the full description below.Full time hours on a temporarybasisKey Responsibilities Care Package Brokerage and Coordination Arrange and record care packages accurately using designated case management systems, ensuring compliance with organisational procedures and quality standards.Source and broker care and support services from contracted providers in line with assessed needs, care plans, and available resources.Manage daily referrals and demand for care services, prioritising urgent cases including hospital discharges and safeguarding situations.Support the coordination of out-of-area placements, ensuring continuity of care and adherence to relevant policies and procedures.Ensure care arrangements are implemented within required timescales to support effective service delivery.
