We are inviting an experienced Band 6 Mental Health Practitioner to join our Community Mental Health Service (CMHS) on the Isle of Wight, where you’ll work with people with complex mental health needs within a supportive multidisciplinary team.

About the Role
You’ll manage a caseload of people with severe and enduring mental health needs, providing assessment, care coordination and evidence-based interventions. You’ll work autonomously while contributing to a multidisciplinary team, supporting junior colleagues and helping shape high-quality community mental health care.
This is a time to join CMHS as we develop our locality model and work more closely with physical health services to provide integrated support.

Main duties of the job
As a Band 6 Mental Health Practitioner in CMHS, you will play a role in delivering high-quality, recovery-focused care to people with severe and enduring mental health needs in the community. Your experience and clinical judgement will make a real difference to service users, carers and colleagues
- Manage a caseload of people with complex mental health needs, taking responsibility for care coordination and treatment.
- Undertake comprehensive assessments and develop, implement and review evidence-based care plans.
- Assess and manage risk, including responding effectively to crisis and urgent situations.
- Provide individual interventions and support people towards their recovery goals.
- Participate in triage and assessment of new referrals into the CMHT.
- Work autonomously while contributing to a multidisciplinary team.
- Provide clinical advice, consultation and support to junior colleagues, including during relapse or crisis.
- Supervise junior multidisciplinary staff and support their professional development.
- Deputise for the Clinical Team Leader when required.
- Build effective relationships with service users, carers, families and partner agencies.
- Contribute to audit, service evaluation, quality improvement and service development.
- Participate in teaching, mentoring and practice development.
- Undertake duty functions and community appointments as required.

Qualifications
Essential
- RN (MH) with live NMC registration or OT or Social worker with current professional registration.
Experience
Essential
- Evidence of on-going development and post registration training.
- Post registration training in evidence based psychological treatments.
- Demonstrable Care Co-Ordinator Competencies.
- Training in Risk assessment and management skills.
- Ability to build on the knowledge, skills and priorities of people working in a range of disciplines.
- Experience in the assessment and management of people with serious and enduring mental health.
- Evidence of undertaking audit work.
- Innovative, creative and a willingness to try new ideas.
- Nurse Mentorship Qualification or relevant professional equivalent.
