Midlands Partnership NHS Foundation Trust is seeking a Mental Health Practitioner (Nurse/OT/Social Worker) to join a 24/7 Crisis Resolution Home Team based at Hall Court, Telford. Deliver high-quality clinical care, assessments and home treatments within an integrated multi-disciplinary service.
The post is Band 6, permanent, full-time with 33.75 hours weekly across 3 x 11h 15m shifts plus a 7h30m shift. Applicants must hold UK registration.
