Hospital Discharge & Care Transitions Lead

Wiltshire Council is seeking a Locality Coordinator to join the Wiltshire Locality Hub, coordinating hospital discharges and working closely with hospital teams and Flow Services to ensure safe, timely transitions from hospital. The role operates seven days a week and requires participation in a weekend rota.

A background in health or social care and experience supporting adults to live independently at home are valuable assets.
