Geriatric OT — Community Care (2 days/week)

Boston Primary Care Network in Boston, Lincolnshire is seeking an enthusiastic Occupational Therapist to join our Primary Care Wellbeing team on a 2-day-per-week job-share. The role focuses on older adults living in care homes, working with EHCH care coordinators, neighbourhood nurse, the acute trust and care homes.

You will assess and treat frail elderly patients in various settings, plan patient-centred interventions, support MDT meetings, and contribute to service development within the PCN.
