Flexible Rehab Assistant – Home-Based Community Care

Calderdale and Huddersfield NHS Foundation Trust is seeking a Rehabilitation Assistant (NHS AfC Band 3) for community-based care in Halifax. The role is part-time with flexible working, supporting adults at home and across Calderdale.

You will participate in rapid response and rehabilitation to promote independence and safe discharges from hospital. The post offers professional development, a seven-day service via rota, and collaboration with a multi-disciplinary team in a community setting.
