Plymouth Hospital Discharge Social Worker – Hybrid

Eden Brown Synergy is seeking a Social Worker to join the Hospital Discharge Team at Derriford Hospital, Plymouth. Hybrid working and competitive pay up to £35 per hour.

The role involves supporting discharges for people with complex social care needs and completing follow-up reviews in the community, within a MDT including nurses and health staff. You should be able to work independently and as part of a team.
