Homeless Pathway Navigator

Single Homeless Project in London is seeking a Homeless Pathway Coordinator to support patients experiencing homelessness, guiding them through health, housing and support services on wards and in A&E. You will manage a caseload, complete assessments, develop clear discharge and support plans, and make referrals across housing, healthcare, social care, mental health and substance use services.
