Autonomous Community Mental Health Clinician

This position is within the Mental Health Services for Older People (MHSOP) Community Mental Health Team. You will work autonomously, applying robust clinical assessment skills to develop and deliver evidence-based care plans for older adults with complex mental health needs.

As a Community Mental Health Practitioner, you will assess risks, coordinate care with families and carers, and collaborate with a multidisciplinary team to provide person-centred interventions.
