Referral Coordinator — Impactful Care Operations

Achieve Together is seeking a Referrals Coordinator to support our community hub team in the UK. The role involves managing the referral journey from initial enquiry through to admission, maintaining accurate records, funding details and admission paperwork, and coordinating with Growth and Partnerships Managers and operations teams to ensure timely delivery of services.

The role requires strong organisational skills, attention to detail, and proficient use of Excel.
