Peritoneal Dialysis Nurse Specialist — Community Care Lead

King’s College Hospital NHS Foundation Trust seeks a Band 6 Community Peritoneal Dialysis Clinical Nurse Specialist to support patients receiving PD at home and in the community. You will deliver expert PD care, educate patients and carers, and work within a multidisciplinary team to maintain patient independence.

You will participate in audits, follow evidence-based guidelines, and liaise with primary care to optimise community-based renal care.
