Senior Rapid Response Nurse: Community Home Care

Joined Up Care Derbyshire is seeking a Senior Community Nurse to join the Rapid Response Nursing Team in Derby City. You will deliver acute care in patients’ homes and residential care settings, with 7 days a week coverage from 08:00 to 18:30, focusing on health education and patient self-management.

You will triage and assess, mentor staff, provide high-quality care for complex cases, and participate in service improvement within a multidisciplinary team.
