Community Mental Health Nurse: Intensive Case Management

Oxford Health is recruiting Community Mental Health Nurses to join a newly established Intensive Case Management and Rehabilitation Pathway across Buckinghamshire. You’ll deliver person‑centred care in people’s homes and community settings, building therapeutic relationships and supporting recovery.

Working within a multidisciplinary team, you’ll assess needs, plan care and provide evidence‑based interventions while contributing to service development and quality improvement.
