Heart Failure Nurse – Hospital at Home & Community

Calderdale and Huddersfield NHS Foundation Trust is recruiting a Community Heart Failure Specialist Nurse to join our expanding Heart Failure team in Halifax. The role supports Band 5/6 nurses transitioning to advanced practice within a collaborative multi-disciplinary team.

Will work across inpatient, outpatient and community settings, with a focus on early identification and education, while contributing to the Hospital at Home service development and delivering high-quality heart failure care.
