Home-Based Community Nurse (Band 5) — Promote Independence

NHS Scotland is seeking a Band 5 Community Nurse to join the Formartine Community Nursing Team, based at Oldmeldrum. You will work flexibly across the Formartine locality, delivering high-quality, person-centred nursing care within the Aberdeenshire Health and Social Care Partnership.

You will manage a defined caseload, promote independence, prevent unnecessary hospital admissions and support patients to remain at home.
