Overview
In this role you will be the first point of contact for public members, patients and professionals, delivering professional, confidential support via phone, email and post. You will signpost and navigate to appropriate community services, manage referrals and maintain accurate clinical records. You’ll work within service pathways to ensure timely referral and triage, while handling sensitive information with care. The position combines administrative rigor with strong interpersonal skills to support patients and colleagues in a busy health environment.

Responsibilities
- act as first point of contact by phone, email and post
- navigate callers to suitable community services with signposting and advice
- manage flow of referrals (paper and electronic) following service pathways
- capture service user contact and follow-up actions in clinical records
- triage and coordinate timely access to appropriate teams or professionals
- maintain confidentiality and handling of sensitive information
