Senior Clinical Pharmacist – Oncology & Haematology

Responsibilities

• Provide expert clinical pharmacy input within the multidisciplinary team
• Conduct medicines reconciliation, clinical screening, and therapeutic drug monitoring
• Optimise medicines use through evidence-based interventions for long-term and acute conditions
• Support prescribing decisions via clinical assessment, risk evaluation, and formulary guidance
• Lead implementation of medicines optimisation strategies aligned with guidelines
• Contribute to development and review of clinical guidelines, protocols, and policies
• Monitor and report medicines usage, safety incidents, and participate in quality improvement

Key requirements

• GPhC-registered Pharmacist
• Significant post-registration clinical pharmacy experience, preferably in hospital
• Strong clinical knowledge in medicines optimisation
• Excellent communication, leadership, and multidisciplinary collaboration skills
• Experience mentoring or supervising junior staff
• Must have BOPA Passport and Clinical Pharmacy Diploma
