Independent Prescriber Pharmacist — PCN Patient Care Lead

Foxhill Medical Centre is seeking a Prescribing Clinical Pharmacist to join the PCN team. You will work across the five practices, providing medicines optimisation and direct patient care, including prescribing where appropriate.

The role requires Independent Prescriber status, CPPE pathway completion, and experience in general practice. You will collaborate with pharmacists, GPs, and wider MDT colleagues to improve patient outcomes.
