Job Description:
University Hospital Southampton NHS Foundation Trust is seeking a Discharge Officer to coordinate timely, safe discharges and support patient flow. You will work with nurses, doctors, therapists, social workers, and community teams to plan and execute discharge from hospital to home or ongoing care.
You will manage a caseload, problem-solve in real time, and ensure clear communication with patients, families and partner organisations to minimise delays and improve patient experience.
