Hospital Discharge Contract Lead – NHS Partnerships

West Sussex County Council’s Adults Social Care Department is seeking an experienced Contract Lead to manage hospital discharge contracts with NHS partners and providers. You will lead on contract management, review arrangements and support ongoing commissioning activity to ensure a range of services for people leaving hospital.
