Bridgwater Bay Primary Care Network seeks a Clinical Pharmacist to join the multi-professional Primary Care Team at the Health & Wellbeing Hub. The role focuses on safe, effective, and cost-efficient medicines use, with opportunities to develop knowledge in medicines optimisation across practice teams.

Responsibilities include independent prescribing, medicines reviews, and supporting the PCN and Somerset ICB plans to reduce medicines waste. Weekend rota work may be required.
