Overview

In this role you support the Virtual Wards and Multidisciplinary Team to deliver proactive community health care coordination. You will handle a wide range of administrative tasks, support MDT processes, and enable effective patient care through data management and liaison with health and social care services. The position offers development through a structured training package and cross-team collaboration within a values‑driven, social enterprise environment. You will work with PCNs to improve joint working and patient outcomes.

Responsibilities

  • Receive referrals from PCN teams and coordinate appropriate responses
  • Liaise with statutory and voluntary services to arrange patient support
  • Plan and organise activities (staff rotas, events, meetings)
  • Assist patients with appointments and incidental contact
  • Manage equipment, servicing, and records; handle petty cash and procurement
  • Contribute to data reporting and audits using structured systems
  • Record interventions in electronic systems and support report generation
  • Provide induction and guidance to new starters; supervise and develop staff as needed
  • Make triage judgments to route referrals to the right service at the right time
  • Support marketing and cross-team collaboration within the Integrated Care System

Key requirements

  • Experience of office procedures within an administration role
  • Experience handling confidential information
  • Proven administrative experience
  • Excellent IT skills (Microsoft Office, Excel)
  • Ability to prioritise, organise and meet deadlines
  • Strong communication and listening skills
  • Experience with data entry and data management
  • Ability to work independently and in a team with some supervisory duties
  • Desirable: experience in Multidisciplinary teams; recruitment, selection, and HR management procedures
