Mental Health Practitioner

Overview

In this role you will provide holistic mental health assessment, diagnosis and treatment within primary care, collaborating with a multidisciplinary team while managing an individual caseload. You will deliver care across acute and routine settings, using diverse consultation methods and actively signposting to external resources. You’ll work autonomously within the clinical team, upholding high standards of patient-focused care and contributing to service improvement.

Responsibilities

• Conduct comprehensive clinical assessments and determine care plans
• Manage an individual patient caseload with accurate care plans reflecting assessments and recommendations
• Plan, coordinate and complete episodes of care in partnership with others
• Deliver care across acquisition and routine settings using various consultation modalities
• Signpost to external resources and psychosocial support as needed
• Maintain high standards of patient-focused care and adhere to policies and protocols
• Engage patients and families to gather feedback and drive improvements
• Work autonomously while collaborating with the multidisciplinary team and delegating/referring when appropriate
• Contribute to learning and service delivery improvements through reflective practice

Key requirements

• Minimum 3 years post-registration experience
• Evidence of continuing professional development
• Clinical skills – mental health
• Clinical leadership skills
• Change management skills and ability to support patients to change lifestyle
• Communication skills, both written and verbal
• Ability to communicate difficult messages to patients and families
• Ability to provide teaching and mentorship in a clinical setting
• Nursing and Midwifery Registered
• Registered Mental Health Professional
• Current UK Professional Registration
