Camhs Crisis Team Mental health practitioner

Overview

In this role you support children and young people in crisis within a 24/7 community service. You will conduct comprehensive assessments, develop formulations and deliver brief interventions, while coordinating high-quality, person-centred care as Lead Professional for home treatment. You work closely with families, carers and partner agencies to promote recovery and safe outcomes. The position combines direct clinical work with leadership in a multidisciplinary team, within a values-driven, supportive culture.

Responsibilities

• Conduct comprehensive mental health assessments and formulate care plans for children and young people in crisis
• Provide brief interventions and risk-informed decisions to support recovery
• Lead and coordinate home treatment for young people, developing and reviewing specialist care plans
• Collaborate with families, carers and partner agencies to ensure timely, effective support
• Assess risk using clinical judgment and maintain safety through risk management
• Communicate effectively within a multidisciplinary team and across services
• Travel across the service area to deliver community-based care
• Support flexible working patterns and align with Trust policies

Key requirements

• Experienced practitioners from nursing, social work or occupational therapy
• Strong communication, assessment and risk management skills
• Ability to work within a multidisciplinary team
• Willingness to travel across the service area
• Access to suitable transport
• Clinical judgement to assess risk and provide interventions
• Experience in crisis care or home treatment (implied)
