This role supports people returning home from hospital to recover safely and regain independence

What youll be doingSupporting people at home after discharge with reablement-focused careAssisting with personal care where needed, while encouraging independenceSupporting daily living skills (washing, dressing, meal prep, mobility, routines)Following care plans and recording notes accuratelyReporting changes in condition, concerns, or risks promptlyWorking respectfully with families and professionals as part of a wider team
What were looking forCaring, reliable and professional approachExperience in home care is helpful, but not essential (full training provided)Good communication and respect for dignity, choice and independenceFull UK driving licence and access to a vehicle (essential for Herefordshire)Right to work in the UK
