Overview

As Wellbeing Practitioner, you help people access non-medical support to improve health and wellbeing. You’ll work across PCN practices with a multidisciplinary team, linking individuals to community services and VCSE organisations. You’ll conduct person-centred wellbeing conversations, create tailored support plans, and support community group development. The role blends direct client work with partnership-building and service improvement to enhance social care within primary care.

Responsibilities

  • Engage with patients referred from GP practices to provide holistic, person-centred guidance
  • Conduct wellbeing conversations and assessments to inform personalised support plans
  • Facilitate access to community groups, activities, and practical support services
  • Build and maintain relationships with local voluntary, community and statutory organisations
  • Manage a varied caseload across three network practices, including face-to-face appointments
  • Collaborate with GPs, practice teams and PCN colleagues
  • Support development and sustainability of local VCSE provision and community groups
  • Maintain accurate records and contribute to service improvement initiatives

Key requirements

  • GCSE English & Maths (or equivalent)
  • Level 3 qualification in health, social care, community development or related field
  • Training in motivational coaching & interviewing or equivalent experience
  • Experience in customer/patient care in healthcare settings
  • Experience supporting people with mental health needs
  • Experience working in partnerships and across organisations
  • Ability to work flexibly in a team or independently
  • Ability to recognise when to refer to qualified health professionals or other agencies (including mental health)
