Primary Care Mental Health Navigator

Tees Esk and Wear Valleys NHS Foundation Trust is seeking a First Contact Mental Health Practitioner for Durham Coast across GP practices within the Primary Care Network. You will work in a growing multidisciplinary service, developing innovative and integrated mental health care for local communities.

The role involves triage, assessments, brief interventions and close collaboration with primary and secondary care teams to ensure timely access to services.
